Lightbulb (Potential) Sticky: Post your replacement via LL or in-store

I'm thinking this would be a good thread to "collect" all of the questions about what phone would be replaced by what. Considering that the lists change over time (even daily), simply having people post what they received as a replacement could indicate an "average".

Please, DO NOT include compensation substitutions (or at least briefly indicate that it is one) as it will be a non-standard substitution. What people typically want to know is "my xyzzy phone is fubar'd and I want to know if it's worth paying LL the $50 to get the substitute phone".

So, please post here
1. In Store (ESRP) or LockLine (ERP). For in-store post where if desired
2. What phone you had (and what symptom/damage)
3. What phone was given (and new or refurbished if known)
4. Date replacement occured (if significantly different from posting date)

MODS, if you feel this is worthy of the sticky, please adjust the title and stick it up top.

in store - sanyo 8200 replaced lg 325. slider problem. (last year). treo 650 replaced samsung a940. bluetooth issue. treo 700p replaced treo 650. treo 650 kept deprograming itself. (both this year).
