The $150 rebate requires a yr contract. You don't add 2yrs, you have 2yrs form the day you get the new phone.
So, I purchase phone March 10, then my 2yr contract starts that day. It doesn't matter if I send the rebate 30 days later or not at all. As long as I request the rebate radioshack will note the rebate was given.

but it says $100 for 1 yr, $150 for 2 yr on the form... its it automatically $150?

no, you could have requested a one year contract and they would have given you a different rebate form to get 100 dollars.

my form says either $100 or $150... it printer out of the computer there. no choice given on what contract

Now that you know what amount goes with what contract, merely select the one that matches...remember, the company doing the rebates is not Sprint (don't care how you feel about Sprint, just doing their job), so it has to be picture perfect.

Here is the standard schpiel I give my customers. I have had bosses tell me not to say it, but a customer needs fair warning...

1. Read everything the form says and be note-perfect, as mentioned above. UPC code is a requirement, and after you cut it off, you cannot return the product, as we have to resell it...fair warning.

2. Write clearly...this is why most people who don't get the rebate fail to, and also why it says to use CAPITAL LETTERS.

3. PHOTOCOPY it after you fill it out but before you send it. If they need you to fax a copy of what you sent, you need to be able to.

...and the one I now omit because it makes the scpiel too long, and I already told them more than anyone else...

4. Get delivery confirmation, so if they say they never go it, you can say "Oh, yes you did, on this date at this time, and here's the confirmation number to verify. HINT: If you go with Priority Mail it may cost more, but confirmation is under a buck...plus it gets better treatment, and is more likely to get there.

Call customer service, they will make sure you are set up properly so the rebate does not get rejected.

To reveive a $150 rebate you need....

The original(old) phone has to have been on your line of service for 18months
A new 2yr Advantage Agreement on your line of service
On a plan of $34.95 or more per month (not encluding attachments)
A Vision plan if the new phone is Vision capable
An account in good standing (not hotlined, suspended etc)
